Membership

Below you will find the information and documents comprising the 2009-10 Illinois HOSA membership packet. The following are necessary in order for membership to be complete:

  1. Chapter Affiliation Form – general chapter information needed. Please include the advisor’s email address on this form.
  2. Chapter Membership Form – complete names of members listed in alphabetical order by last name. The health science course, which the student is enrolled in, and the date enrolled are required. Home addresses of members are not required.
  3. Payment of Membership Fees -- $12 for all members. Do not forget to include professional membership fees with your chapter’s membership. Checks should be made payable to Illinois HOSA. School checks are preferred. A personal check from an advisor is acceptable; however no checks from students and their families or cash are acceptable.
  4. Administrator’s Signature – an administrator must sign the membership form.

A chapter’s membership will not be processed until the above items are completed.

Chapters should submit at least 5 memberships in order to be considered active. If a chapter wishes to receive inactive status, one professional member’s membership fee needs to be paid. Membership affiliation deadlines are listed on the membership form.

A HOSA calendar can be found here. If your chapter creates a calendar, please submit a copy of it with your membership. Documentation of chapter activities is again required to support the expenditures of contract funds.
